1. Chest radiographic findings

• CXR upright position
• Exposure:  Under, due to not seen spinal column
• Inspiration:  Adequate 
• Rotation:  Rotation to right 
• Parenchyma:  Diffuse multiple small nodules (vary in size), ill-defined border, soft tissue density with calcified, predominate at periphery and lower lung zone, pleural area and also seen at left lower lung and upper lung
• Soft tissue:  Normal 
• Bony:  Not seen osteolytic lesion
• Gastric gas: Normal 
• Cardiac: normal cardio thoracic ratio, no significant chamber enlargement, aorta with normal limit, mediastinum with normal limit
• Diaphragm: No blunt costrophrenic angle, below diaphragm normal

2.The most likely diagnosis 

• Clinical correlation:  Case old age male with history of paraplegia and weight loss ผู้ป่วยสามารถยืนถ่ายเอกเรย์ได้ แม้ว่าจะมีพยาธิสภาพในปอดมาก แสดงถึงการดำเนินโรคน่าจะเรื้อรัง
• Provisional Dx:  Secondary carcinomatosis with hematologic spreading
