1.    Differential diagnosis  Sporotrichoid pattern 

• Atypical mycobacterial (M.marinum, M.chelonae, M. Kansasii)
• Nocardiosis
• Pyogenic bacteria (S.aureus, Streptococcus pyogenase)
• Pseudallescheria boydii, Scedosporium apiospermum)

2.    Most likely diagnosis  Sporotrichosis 

3.    Proper management 
- For diagnosis:  Sporotrichosis is best diagnosed by a culture of clinical specimens, molecular methods, such as polymerase chain reaction (PCR).

• Culture: when cultured at 25°C, it assumes a filamentous form, composed of hyaline, septate hyphae 1 to 2 μm wide, with conidiogenous cells arising from undifferentiated hyphae forming conidia in groups on small, clustered denticles
• Histology: a mixed granulomatous and pyogenic process, an asteroid body composed of a central fungal cell   surrounded by Splendore-Hoeppli material

- Treatment

• Itraconazole is the drug of choice for sporotrichosis. 
• Treatment duration is typically at least 1 month after clinical findings resolve.
