A 66-year-old Thai man presented with recurrent transient loss of consciousnes. 

He had history of urinary retention with post-void residual urine volume of more than 100 ml., suspecting neurogenic bladder from autonomic dysfunction.

He had history of ataxia and revealed gait and limb ataxia and cerebellar dysarthria during physical examination.

Questions:
