ANSWERs

1. The most likely diagnosis

• Non-scarring alopecia, ill-defined border 
• Diagnosis:  secondary syphillis 

2. Please give the appropriate laboratory investigations

• Non-treponemal tests:  RPR หรือ VDRL 
• Treponemal:  FTA ABS, TPPA/TPHA, CMIA for syphillis

3. Please give the most effective treatment (drug and dose) 

• Penicillin G 2.4 mU IM single dose, follow up VDRL or RPR at 6 mo.
• Alternative:  
  • Doxycycline (100) 1x2 PO pc x 14 days 
  • Tetracycline (500) 1x4 PO pc x 14 days 
  • Azithromycin (250) 4 tab PO once 
  • Ceftriaxone 1-2 g IV or IM 10-14 days
