A 30-year-old Thai woman with SLE presented with acute febrile illness for 5 days. She was lost to follow up intentionally and had been taking herbal medications for 2 months.

Septic work up result:  UA protein trace, glucose negative, WBC 50-100/HPF, RBC 50-100/HPF, epi 0-1/HPF

Urine analysis by light microscope was as shown above. 

Questions:

1.Urine analysis findings 

• 1. Coffin lid-shaped crystals 
• 2. Magnesium ammonium phosphate crystals (triple phosphate crystals)

2.Provisional diagnosis  Urinary tract infection 

• Crystals ชนิดนี้พบใน urinary tract infection โดยเชื้อที่ทำให้เกิดได้ คือ เชื้อที่มีการสร้าง Urease Enzyme เช่น Proteus, Klebsiella 
• สำหรับผู้ป่วยรายนี้ผล urine culture, hemoculture พบเชื้อ Proteus mirabilis
