A 76-year-old Thai woman, with HT, old CVA, and tracheomalacia on tracheostomy tube, presented with nausea and vomitting and shortness of breath for 1 hour PTA. 

• Previous status: she could walk a few steps with accompanying persons 
• At the ER, the patient seemed agitated, fidgety, incline, sloping and was not obeying to command. Her blood pressure was 60/- mmHg. 
• CXR was done and the result was shown above, compared with baseline CXR in 2015. 

Please interpret the current CXR and give the provisional diagnosis
