A 75-year-old woman presented with recurrent skin lesions at legs for 2 years. She had been treated as chronic eczema with systemic corticosteroids but the lesions still persisted. 

• Underlying diseases: 
  • 1.CA breast S/P MRM, received adjuvant therapy with doxorubicin and cyclophosphamide, then hormonal treatment for 5 years (until Nov 2020), now undergoing surveillance for cancer 
  • 2.Chronic hepatitis B infection 

Questions: 

1.Please describe the skin lesions and give differential diagnosis 

2.What would be the most proper next step investigation?

ANSWERs

1.Describe the skin lesions and give differential diagnosis 

• Well-demarcated erythematous plaques with variable sizes and shapes, occasionally accompanied by poikiloderma, on the upper and lower extremities, trunk, and back, with pruritus
• Differential diagnosis includes lymphoma, psoriasis, chronic eczema 

2.The most proper next step investigation

• Skin biopsy for tissue pathology 
• Staging CT chest and whole andomen
• BM study

This patient had undergone incisional skin biopsy. The result revealed positive staining for CD3 (diffuse), cyclin D1 (diffuse), CD30 (focal in large lymphoid cells), Bcl-2 (diffuse), MUM-1 (patchy), negative for CD20, CD10, ALK, Bcl-6, and Ki-67 70%.

The final diagnosis was Cutaneous T-cell lymphoproliferative disease. 

Pending additional IHC studies included CD4+, CD8-, EBER-, CD56-, Granzyme B -/+
