A 47-year-old woman, without an underlying disease, presented with insidious onset of blurred vision of the Rt eye for 8 months. The color and night vision got worse overtime and she had trouble driving at night. She occassionally had light headache without obvious neurological deficits. 

• Eye examinations: 
  • VA RE 6/60+2, LE 6/6
  • Both eyes showed Tn 18, no injected conjunctiva, clear cornea, deep A/C with no cells, C:D 0.4 pink. 
  • Fundus examination showed attenuation of vessels at Rt inferotemporal region, otherwise normal. 
• Visual field examination and MRI brain and orbit were performed.

2.What is the provisional diagnosis? 

3.What are the proper next step investigations and management?

1.Abnormal findings of the MRI brain

• Coronal contrasted T1W:  A well-defined extraaxial mass in the tuberculum sellae, size 2.0x1.9x1.7 cm with dura tail sign, displacing bilateral A1ACAs
• Sagittal non-contrasted T1W:  hyperintensity spot at posterior pituitary region

2.Provisional diagnosis  Meningioma 

3.Proper next step investigations and management

• Work up pituitary hormones 
  • Prolactin IGF-1 FSH LH Testosterone or Estrogen FT4 TSH Serum cortisol ACTH Electrolyte Urine spec
• Consult Neurosurgery
