A 47-year-old Thai man, no known underlying disease with illicit drug use, presented with dyspnea, bilateral leg edema, and ascites for 1 month. Diagnosis was right sided heart failure.

• During hospitalization, he developed tachyarrhythmia. EKG was done, as above. 
• Adenosine 6 mg and 12 mg intravenous were given respectively.
• Afterwards, his EKG did not show a significant change.

ANSWERs 

Diagnosis: Atrial flutter with 2:1 AV block

Management: In this case, atrial flutter was developed from heart failure, thus we have to decongest first. However, you can consider giving antiarrhythmic drugs, such as amiodarone to control heart rate.
