Chest wall

Case:  A 77-year-old Thai man, with underlying diseases of CAD s/p PCI and BPH, presented with progressive dyspnea with constitutional symptoms for 1 month. 

• Physical examination revealed pale conjunctivae with multiple cutaneous lesions at scalp and chest wall. 
• From the investigations, CBC revealed WBC 5,200 N 3% L 52% Mono 10% Promyelocyte 24% Blast 10%, Hb 6.4, Hct 18.5% and Plt 165,000. 

Questions: 
1. Please describe abnormal skin lesions from the images above. 
2. What is the provisional diagnosis?

ANSWERs 

1.Description of abnormal skin lesions

• Numerous discrete skin-color to erythematous papules and nodules on scalp, face, neck, and upper trunk

2.Provisional diagnosis

• จากประวัติที่ผู้ป่วยรายนี้มาด้วย Progressive dyspnea with constitutional symptom ร่วมกับตรวจร่างกายมีภาวะซีด ส่งตรวจทางห้องปฏิบัติการ พบภาวะ Anemia ตรวจพบ Immature WBC จำนวนมาก 
• Peripheral blood smear พบ immature WBC ลักษณะขนาดใหญ่ เห็น Cytoplasm ค่อนข้างมาก Smooth chromatin ซึ่งเข้าได้กับ Myeloblast 
• ในผู้ป่วยรายนี้เข้าได้กับ Leukemia cutis จาก Acute Myeloid Leukemia (AML)
• ซึ่ง Leukemia cutis เป็น Rare skin presentation ของ Leukemia เกิดจาก Leukemic cell infiltration สัมพันธ์กับ Aggressive disease และ Poor prognosis สามารถพบได้ทั้งใน AML, CML, ALL, CLL ซึ่งสามารถแยกกันได้ด้วยการทำ Skin biopsy และส่งตรวจ Immunohistochemistry markers  
• การรักษา : No specific treatment, Systemic leukemia therapy
